Dr. David Burns book "Feeling Good"

I'm a hundred or so pages into Dr. Burns' book. It is not only practical it is a fun read (in the genre of depression). The forms he suggests to record and analyze cognitions are potentially quite useful but not available, at least to my ability. .

These forms are called Figures in the book and are, for the most part, filled in in the book. In addition, the book I have is 8 x 5" so trying to enlarge will not work very well.

Please let me and the green know if you have a source for Dr. Burns' blank forms.
